letzten Behandlungsdrittel, werden individuell mit den Rehabilitanden die notwendigen Nachfolgema\u00dfnahmen eruiert, geplant und eingeleitet.<\/p>\n<p>Falls bereits eine Anbindung an eine wohnortnahe Suchtberatungsstelle des Rehabilitanden besteht, werden Kontakte w\u00e4hrend der Behandlung gef\u00f6rdert und begleitet und mit der Beratungsstelle gemeinsam die weitere Planung und Anbahnung von Unterst\u00fctzungsma\u00dfnahmen vorgenommen. Im Falle eines anstehenden Ortswechsels f\u00fcr den Rehabilitanden wird mit ihm gemeinsam eine Vermittlung an das Suchthilfesystem vor Ort vorgenommen. Eine weitere Unterst\u00fctzung in einer Suchtberatungsstelle kann zum Beispiel im Rahmen der ambulanten Nachsorge erfolgen. Eine ambulante Weiterbehandlung (40+4) kann dar\u00fcber hinaus einen noch offenen Behandlungsbedarf abdecken.<\/p>\n<\/div><\/section><\/div><\/div><!--close column table wrapper.
